At Easterseals PORT Health, we’re seeking a compassionate and dedicated full-time Behavioral Healthcare Manager to join our Tailored Care Management Team (TCM). As a valued member of our collaborative and supportive team, you will work together to transform the lives of individuals by providing a comprehensive range of services that treat the whole person and empower patients to find their way forward in Beaufort county and surrounding areas.
*$2500 sign-on bonus
What You’ll Do
As a Behavioral Healthcare Manager (BCM) on our team, you will contribute significantly to the well-being of the individuals under your care by coordinating their individualized needs in conjunction with their full care team. The BCM’s focus on closing gaps for physical and mental health by spending time with their clients in their homes or in the community assessing their needs and then making recommendations for services. The TCM team at Easterseals PORT Health care’s directly for our clients in one-on-one sessions on a monthly basis. This position is community-based and travels daily within Beaufort county and surrounding areas.

What We’re Looking For
To join our team as a Behavioral Healthcare Manager you must be able to provide services in various community locations as needed, have working knowledge of Microsoft office suite, experience with Electronic Health Records software, have a passion for helping others and be adept at linking clients to community resources. We also require the following:
- A Bachelor’s degree in health, psychology, sociology, social work, nursing, or another relevant human services area; Master’s preferred
- Able to be credentialed as a Mental Health Qualified Professional
- Minimum of 2 years of experience with the mental health and/or substance use disorder population
- A valid driver’s license, current auto insurance and a good driving record
- Must be able to work independently, have strong verbal and written communication skills
