At Easterseals PORT Health (ESPH), we’re seeking a compassionate and dedicated full-time Director to join our Tailored Care Management team (TCM) in Eastern North Carolina. As a valued member of our collaborative and supportive team, you will work together to transform the lives of individuals by providing a comprehensive range of services that treat the whole person and empower patients to find their way forward.
What You’ll Do
As the TCM Director, you will be responsible for staff management and team training, quality improvement and compliance as defined by the MCO’s and the NCQA Care Management Criteria in addition to monitoring the budget and administrative policies. The Director leads the team in their efforts to ensure growth, consistency, and continued financial viability of the TCM program. The ability to multi-task and think outside the box are critical skills to success in this integral team leadership position.
How You’ll Benefit
Being part of our team means we value and encourage your personal growth and development. You’ll earn a competitive annual salary between $75,000 and 85,000 depending on skills and experience. This full-time position offers a hybrid schedule Monday-Friday primarily 8 am-5 pm with flexibility to support your teams outside of regular business hours, if needed. What We’re Looking For
To join our team as the TCM Director, IDEAL candidates will have experience with clinical coordination, community resources and care plan development in addition to possessing top-tier communication skills, being adept at problem solving and de-escalation in times of crisis. We also require:
- Bachelor’s degree in Human Services and 3 years’ experience in MH/SUD, clinical work and/or care management
- Master’s degree and clinical mental health licensure preferred
