Easterseals PORT Health (ESPH) is seeking an Employment Specialist / Qualified Professional to join our Assertive Community Treatment (ACT) Team. In this role, you’ll help individuals living with severe mental health and substance use challenges find and maintain employment, empowering them to regain independence. - Community Impact: Provide hands-on support in community settings to help clients find and sustain employment, building pathways to independence.
- Collaborative Care: Collaborate with a skilled team of psychiatrists, nurses, therapists, and other mental health professionals to address clients’ comprehensive needs.
- Advocacy & Empowerment: Help clients navigate vocational opportunities and advocate for their rights with compassion and respect.
- Professional Growth: Receive ongoing training, and career development opportunities.
What You’ll Do:
- Engage clients about job opportunities and conduct vocational assessments.
- Build relationships with local businesses for job placements.
- Provide job coaching and support clients in navigating the workplace.
- Develop individualized vocational plans in collaboration with the ACT team.
- Participate in daily team meetings to ensure quality service delivery.
- Maintain accurate and timely documentation using Welligent EMR.
- Offer on-call crisis intervention services as part of a rotating schedule.
Qualifications:
- Bachelor’s degree (in human services or related field preferred).
- Minimum 2 years’ experience working with the adult mental health population – ACT team experience preferred.
- Able to be credentialed as a Mental Health Qualified Professional or Associate Professional
- Comfortable working in community settings with a valid driver’s license.

- Hourly Salary: $43,680 to $54,000/year | $21.00 – $26.45/hour, based on experience
