Easterseals PORT Health is seeking a compassionate, caring, and dedicated Housing Specialist / Qualified Professional to join our collaborative, multidisciplinary, Assertive Community Treatment team (ACT) designed to carry out a full range of community-based treatment functions for adults.
What You’ll Do
The housing specialist is an integral member of the multidisciplinary team and has the responsibility to carry out rehabilitative services intended to increase and restore an individual’s ability to live successfully in the community by maintaining tenancy. The housing specialist will also work with individuals participating in the Transition to Community Living Initiative (TCLI). This member will also provide rehabilitation and support functions and assist in treatment; substance abuse services; education, support, and consultation to families; and crisis intervention under the clinical supervision of staff with professional degrees. You will also:
- Provide case management for an assigned group of clients including coordinating and monitoring the activities of the individual treatment team.
- Assume primary responsibility for developing, writing, implementing, evaluating, and revising overall treatment goals and plans in conjunction with the Individualized Treatment Team (ITT);
- Participate in the provision of structured rehabilitative interventions to individuals served such as housing/tenancy sustaining services, money management and entitlements, activities of daily living, personal health, wellness and recovery, and community integration.
- Coordinate communication with guardians, care coordination, housing stakeholders, landlords, and other agencies serving the individuals being supported.
- Develop relationships with agencies that provide housing services, i.e., Housing Authority, permanent supportive housing providers, rapid re-housing providers, HUD-VASH, NC Housing Finance Agency, and relationships with local landlords to increase access to appropriate, safe, and affordable housing
- Provide on-call intervention covering after hours crisis needs on a rotating schedule.
- Documents client progress to maintain a permanent record of client activity according to established methods and procedures.
- Participates in daily staff organizational meetings and treatment planning review meetings.
How You’ll Benefit
You’ll earn a competitive salary based on your experience between $40,000-$50,000 depending on education, experience, and licensure plus enjoy a Monday through Friday work schedule. What We’re Looking For
To join our team, you must be willing to provide services in community locations, have a passion for helping others, and be able to link clients to community resources. We also require the following:
- A Bachelor’s degree
- Able to be credentialed as a Mental Health Qualified Professional or Associate Professional
- Minimum of 2 years of experience with the adult mental health population-internships included; ACT team experience preferred
- A valid driver’s license, current auto insurance and a good driving record
