Easterseals PORT Health is looking for a dedicated Housing Specialist / Qualified Professional to join our Assertive Community Treatment (ACT) team in Raleigh, NC. This is a full-time, community-based position that offers the opportunity to make a significant impact on the lives of individuals living with severe and persistent mental health conditions.
What You’ll Do
As a Housing Specialist, you will be an integral part of our multidisciplinary team, working directly with individuals to ensure they maintain stable and safe housing. You’ll also work with individuals participating in the Transition to Community Living Initiative (TCLI). Your responsibilities will include:
- Case Management: Help assigned group of clients, including monitoring the activities of the individual treatment team; develop, implement, evaluate, and revise treatment goals and plans.
- Rehabilitative Interventions: Provide structured rehabilitative interventions such as housing/tenancy sustaining services, money management, daily living activities, personal health, wellness, and community integration.
- Support and Advocacy: Build relationships with housing service providers, landlords, and other agencies to increase access to safe and affordable housing. Advocate for individuals’ rights and preferences.
- Therapeutic Support: Offer individual and group supportive therapy and symptom management skills.
- Family Education: Educate and support individuals’ families and advocate for their rights and preferences.
- Crisis Intervention: Provide on-call intervention for after-hours crisis needs on a rotating schedule.
What We’re Looking For
To join our team as a Housing Specialist, you must be willing to provide services in community locations, have a passion for helping others, and be able to link clients to community resources. You should have:
- A strong commitment to the rights and ability of each person living with severe and persistent mental health conditions to live successfully in the community.
- Knowledge of community housing resources.
- Skills and competence to establish supportive, trusting relationships with individuals.
- Respect for individuals’ rights and personal preferences in treatment.
- A valid, unrestricted driver’s license for the State of North Carolina.
- A bachelor’s degree in a related human services field and a minimum of 2 years’ experience with the adult mental health population.
