Housing Specialist / Qualified Professional

Easterseals PORT Health (ESPH) is seeking a Housing Specialist / Qualified Professional to join our Assertive Community Treatment (ACT) Team. You’ll be the key to ensuring clients find and maintain safe, supportive housing while managing their mental health and substance use challenges.   • Community Impact: Provide hands-on support in community settings for client housing stability and personal wellness.
  • Holistic Care: Collaborate with a skilled team of psychiatrists, nurses, therapists, and other mental health professionals to address clients’ comprehensive needs.

What You’ll Do:

  • Provide case management for clients and coordinate housing services.
  • Help clients access housing resources, such as HUD programs and supportive housing.
  • Build relationships with landlords and housing agencies.
  • Develop individualized housing plans in collaboration with the ACT team.
  • Provide support on housing stability, health, daily living, and finances.
  • Participate in daily team meetings to ensure quality service delivery.
  • Maintain accurate and timely documentation using Welligent EMR.
  • Offer on-call crisis intervention services as part of a rotating schedule.

Qualifications:

  • Bachelor’s degree (in human services or related field preferred).
  • Minimum 2 years’ experience working with the adult mental health population – ACT team experience preferred.
  • Able to be credentialed as a Mental Health Qualified Professional or Associate Professional
  • Comfortable working in community settings with a valid driver’s license.

    • Hourly Salary: $43,680 to $54,000/year | $21.00 – $26.45/hour, based on experience
