At Easterseals PORT Health (ESPH), we’re seeking a compassionate and dedicated full-time Qualified Professional to join our Intensive in Home (IIH). As a valued member of our collaborative and supportive team, you will work together to transform the lives of individuals by providing a comprehensive range of services that treat the whole person and empower patients to find their way forward.
What You’ll Do
As a Intensive in-Home Qualified Professional, you will be responsible for providing crisis and case management and individual/family therapy to prevent the need for out of home placement for children with a diagnosed mental health disorder. As a Qualified Professional, you will deliver services in various environments, such as homes, schools, detention centers and jails (state funds only), homeless shelters, street locations, etc. You will also be responsible for coordinating the initial assessment and developing the youth’s Person-Centered Plan as well as diffuse crises, evaluate nature of crises, and intervene to reduce the likelihood of a recurrence.

This full-time position offers a flexible 40-hour nontraditional schedule and will serve the Iredell County and Davie Counties. We offer competitive benefits to benefits eligible positions.

What We’re Looking For
To join our team as a Qualified Professional, you must be willing to provide care in various community settings, rotate first responder coverage, have a passion for helping others, have experience with diffusing crisis and able to link clients to community resources. We also require the following:
- Bachelor’s degree with 2 years post-degree experience with at-risk youth population
- Or a Master’s degree with 1-year post-degree experience with at-risk youth population
- A valid driver’s license, current auto insurance and a good driving record
