What You’ll Do
As a Tailored Care Management Extender on our team, you will focuses on outreach and engagement to assigned Tailored Care Management members in an effort to support the individual’s ability to live as independently as possible, managing their recovery, and reestablishing their community roles related to the following life domains: emotional, social, safety, housing, mental and physical health, educational, vocational, and legal.

What We’re Looking For
To join our team as a Tailored Care Management Extender you must be able to provide services in various community locations as needed, have working knowledge of Microsoft office suite, experience with Electronic Health Records software, have a passion for helping others and be adept at linking clients to community resources. We also require the following:
- A minimum of a high school diploma or equivalency required from an approved, accredited institution.
- Two years of paid experience performing the types of functions above, with at least one year of paid experience working directly with the Tailored Care Management eligible population.
- Must have current, unrestricted, NC driver’s license.
- Must have current, unrestricted vehicle insurance and provide own transportation for use in fulfilling job requirements.
