Tailored Care Manager

At Easterseals PORT Health, we’re seeking a compassionate and dedicated full-time Tailored Care Manager to join our Tailored Care Management (TCM) team in the Onslow County. As a valued member of our collaborative and supportive team, you will work together to transform the lives of individuals by providing a comprehensive range of services that treat the whole person and empower patients to find their way forward.

*$1500 Sign on Bonus

What You’ll Do 

As a Tailored Care Manager, you will be responsible for promoting whole person, integrated health care through evaluation, assessment, care planning, coordinating, tracking, and monitoring outcomes for individuals served. The focus will be on closing gaps in care for physical and mental health, social determinants of health, and other non-health related needs. The service area of this opportunity is Onslow county.

You’ll earn a competitive salary $52,000 and 55,000 based on your experience, plus enjoy a Monday-Friday, 8am-5 pm work schedule with some evening flexibility for program and client needs. What We’re Looking For

To join our team as a Tailored Care Manager you must be able to provide services in various community locations as needed, have working knowledge of Microsoft office suite, experience with Electronic Health Records software, have a passion for helping others and be adept at linking clients to community resources. We also require the following:

  • A Bachelor’s degree in health, psychology, sociology, social work, nursing, or another relevant human services area; Master’s preferred
  • Able to be credentialed as a Mental Health Qualified Professional
  • Minimum of 2 years of experience with the mental health and/or substance use disorder population
  • A valid driver’s license, current auto insurance and a good driving record
  • Must be able to work independently, have strong verbal and written communication skills
