Youth Support Partner (High Fidelity Wraparound)

At Easterseals PORT Health (ESPH), we’re seeking a compassionate and dedicated full-time Youth Partner (YP) to join our team in Greensboro, NC. As a valued member of our collaborative and supportive team, you will work together to transform the lives of individuals by providing a comprehensive range of services that treat the whole person and empower patients to find their way forward.

This amazing position has a few requirements that you must meet in order to be considered. 

  • You must be between the age of 21-32 (State Requirement for this unique role)
  • Must have lived experience as an adolescent who personally has received services for mental health or substance use challenges while under the age of 21 (Work experience will not be considered) 

What You’ll Do

The Youth Support Partner provides one-on-one structured supports and strengths-based interventions that promote self-determination and self-advocacy with a focus on recovery and resiliency for the benefit of the eligible youth. Supports provided to the youth are directed towards positive outcomes and to increase the overall capacity for the youth to build healthy, positive relationships in the here and now that will carry into their future. The Youth Partner meets with the youth on a weekly basis as well as with the family and support system, collaborates with the whole care team, and supports the youth by assisting them with outside agencies and community referrals as needed.

What We’re Looking For

To join our team, you must be willing to provide care in community and home-based settings, participate in the crisis phone support rotation, have strong communication skills and able to link clients to community resources. We also require the following:

  • Must have lived experience as an adolescent who personally has received services for mental health or substance use challenges while under the age of 21
  • Required education:
    • Minimum of a Bachelor’s degree or
    • An Associate’s degree in Human Services field AND 1-year of experience working with youths and adolescents, OR
    • A high school diploma or GED AND a minimum of 2-year’s of experience working with youths and adolescents
  • A valid driver’s license, current auto insurance and a good driving record
  • Experience with navigating child and family servings systems
