Easterseals PORT Health’s Developmental Development (DD) Peer Support Mentors promote self-determination and support as individuals are making life choices. We provide advocacy and help individuals with developmental disabilities identify social determinants of health that need resources in order to become a part of their community.

Specialized Consultative Services

The Individual and Community Services team supports individuals with developmental disabilities and severe emotionally disturbed/serious mental illness (DD and SED/SMI). We provide one-to-one and group services uniquely tailored to help each unique family acquire and maintain resources for social determinants of health. We provide services such as:

  • Activities and social engagement.
  • Community collaboration and outreach.
  • Resources and information.
  • Supports and empowerment.
  • Support groups.
