Supporting young people with substance use disorders
Easterseals PORT Health’s Teen Substance Use Program provides residential and day treatment care. Our program is for teens who have not been successful with other interventions and require 24-hour supervised care. While in treatment, teens participate in specific therapies to address their needs.
Our onsite Teen Residential Treatment Program includes:
- A combination of day treatment and residential services in a highly structured environment with 24/7 staff engagement and supervision.
- Daily individual and group therapy using evidenced-based best practices in a structured program.
- Monday through Friday onsite academic classes.
- Family engagement that includes phone calls, visitations and regular meetings with the treatment team.
- Psychiatric evaluations and medication management as needed.
- Introduction to community-based recovery resources.
- Participation in daily therapeutic activities such as art, physical activity (including exercise and access to recreational equipment), sports, yoga, music therapy, community outings and recreational activities.
- Teaching behavior modification through participation in activities of daily living.
What is the referral process?
Referrals can be made by anyone working with or caring for the adolescent. However, it’s recommended that referrals be completed by a professional who is well-acquainted with the adolescent’s treatment history and current needs. Our team will respond to referrals as quickly as possible. Please be patient for a response. Our support services are in high demand and bed space is often limited.
Once eligibility for admission is determined, the following items are required prior to admission.
- Completed online referral application
- Proof of current Medicaid or private insurance (if applicable).
- Copy of treatment documents that support the teen’s need for admission (i.e., discharge summaries from recent placements, most current assessment, psychological testing, etc.)
- Active court documentation.
- School records including IEP, 504 and other information to support the teen’s ongoing education while in treatment.
- Tuberculosis (TB) skin test, proof of immunizations and physical exam records.
- Birth certificate.
- Relevant medical documentation of any pre-existing medical conditions that require monitoring while in treatment.
- 30-day supply of medications upon admission.
